The Risks of Fixing Errors Too Late
For many risk adjustment leaders, retrospective coding feels like the safety net.
Charts get reviewed after the fact. Missed HCCs get flagged. Audits are performed. Cleanup cycles are put in place to make sure nothing slips through the cracks. On the surface, it feels responsible. Even reassuring.
But over time, many organizations start to notice a troubling pattern. Despite increasing review volume and layering on more checks, revenue recovery gets harder, not easier. RADV risk doesn’t go away. Documentation gaps persist. And the effort required to hold the line keeps growing every year.
At some point, it becomes fair to ask a difficult question:
What if the problem isn’t how well we’re reviewing charts – but when we’re doing it?
The comfort of “we’ll catch it later”
CMS guidance has understandably shaped how risk adjustment programs operate. The priority has been clear: don’t submit codes that aren’t supported by the medical record. Avoid false positives. Minimize audit exposure.
That emphasis matters. But many organizations have taken the next step without realizing it. They’ve built workflows that assume anything important can be fixed downstream.
If a diagnosis is missed, retrospective review will catch it.
If specificity is lacking, a later pass will correct it.
If documentation isn’t clear, someone will reconstruct the story later.
For a while, this works well enough. But slowly, the cracks start to show.
When effort increases but outcomes don’t
One of the most common frustrations among risk adjustment leaders is this: review volumes keep climbing, but results don’t improve at the same rate.
More charts are touched. More time is spent. More handoffs are introduced. And yet, some missed HCCs never come back. Some specificity is never recovered. Some opportunities simply disappear.
That’s because retrospective workflows come with real constraints that aren’t always obvious at first.
Once the encounter window has passed, providers are no longer engaged in the same way. Clinical context fades. Subtle documentation details are harder to interpret. Clarifications that might have been simple earlier become impractical or impossible.
At that point, coding becomes reconstruction rather than confirmation. And reconstruction is always harder to defend.
Why timing quietly determines what’s recoverable
In risk adjustment, timing is not a minor detail. It fundamentally shapes what can be captured, supported, and defended.
The later an issue is identified, the more likely it is that:
- Documentation context has been lost
- Provider engagement is no longer feasible
- Specificity has to be inferred rather than confirmed
- Audit defensibility becomes more fragile
This is where many teams feel a growing sense of unease. They’re doing the right things operationally, but the workflow itself is working against them.
Retrospective coding creates a sense of control. It feels like a safety net. But in reality, it often allows revenue leakage to compound quietly, one missed opportunity at a time.
The false promise of “just review more”
When results stall, the instinctive response is to add more review. Expand the chase list. Another audit layer. Another pass. Another cleanup cycle.
But more reviews don’t necessarily mean better outcomes. They often mean higher labor costs, more handoffs, and more operational drag. And critically, they don’t change the fact that some errors are only visible after the window to fix them has closed.
Where technology can reinforce the problem
Many organizations have leaned on automation and machine learning tools to support retrospective workflows. These tools can increase speed, but they don’t change the underlying issue.
Machine learning systems often predict what might be true based on patterns. In a CMS-regulated environment, that introduces risk. Diagnoses inferred statistically are harder to defend than diagnoses directly tied to explicit documentation. Highly specific, combination, complex and rare ICDs are often missed. Black-box logic makes it difficult for coding leaders, compliance teams, and auditors to understand why a code was selected. Additionally, machine learning requires weeks to months of training each year with CMS required updates. This leads to more time lost and increased financial impact.
When those tools are applied late in the process, they amplify the core problem: you’re still trying to fix things long after the fact.
The realization many leaders eventually reach
At some point, risk adjustment leaders start asking different questions.
Why does it feel like we’re working harder just to stay in place?
Why do audits still feel stressful even with all these controls?
Why does revenue recovery feel less predictable every year?
The answer is uncomfortable but clarifying. The issue isn’t execution. It’s timing.
By the time charts are reviewed retrospectively, some errors are already unrecoverable. Some specificity is already gone. Some revenue is already lost.
The one idea worth holding onto
Retrospective coding doesn’t just delay fixes. It quietly limits what can realistically be fixed at all.
Recognizing that reality doesn’t mean abandoning compliance or quality. It means questioning whether workflows designed around “fixing it later” are unintentionally working against the very outcomes they’re meant to protect.
For many organizations, that realization becomes the turning point. Not toward moving faster for the sake of speed, but toward finding earlier visibility – while opportunity and defensibility still exist.
